Our sliding scale fee structure is based on your household income:

| $60,000 – $79,999 | $110 |
| $80,000 – $99,999 | $120 |
| $100,000 – $119,999 | $125 |
| Above $120,000 | $150 |
You have the right to receive a “Good Faith Estimate” explaining how much your care will cost.
Under the No Surprises Act, health care providers must give clients who don’t have insurance — or who choose not to use it — an estimate of expected costs before services begin.
- You have the right to a Good Faith Estimate for the total expected cost of any non-emergency services.
- You can ask for a Good Faith Estimate, in writing, before you schedule a service.
- If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute it.
- Keep a copy of your Good Faith Estimate.
